Key Responsibilities

• Provide occupational therapy services to patients in a skilled nursing facility setting.
• Conduct assessments, develop treatment plans, and implement therapy interventions.
• Document patient progress and coordinate care with healthcare team members.
• Ensure compliance with all state and facility-specific regulatory requirements.
• Maintain accurate licensure, credentialing, and documentation for all therapy services provided.

Required Qualifications

• Valid occupational therapy license for the state of Oregon.
• Willingness to complete and submit prescreen documentation, including resume, license verification, and references.
• Ability to pass required background checks and screenings, including criminal and sex offender searches.
• Covid-19 test administered within 72 hours prior to start, with results provided within 48 hours.

Preferred Experience / Skills

• Prior experience in skilled nursing facilities (SNF).
• New graduates are welcome to apply.
• Experience with documentation and adherence to regulatory credentialing processes.
