Key Responsibilities

• Provide occupational therapy services to patients in a skilled nursing facility setting.
• Conduct assessments and develop individualized treatment plans to meet patient needs.
• Document patient progress and communicate with healthcare team members.
• Ensure compliance with all licensing, credentialing, and safety protocols.
• Maintain accurate records and complete required documentation for credentialing and vaccination.
• Collaborate with staff to promote optimal patient outcomes.

Required Qualifications

• Valid state occupational therapist license.
• Ability to complete and verify credentialing requirements, including background checks and health screenings.
• Current CPR certification.
• Successful completion of physical and drug screening tests.
• COVID-19 test within 72 hours prior to starting with results issued within 48 hours.
• 8 hours of dementia-specific training.

Preferred Experience / Skills

• Experience in a skilled nursing facility or SNF setting.
• Rehab Optima experience preferred.
• Recent COVID-19 vaccination.
• New graduates are encouraged to apply.
