Key Responsibilities
- Provide direct patient care in a Long-Term Care (LTC) or Skilled Nursing Facility (SNF) setting.
- Administer medications and monitor patient conditions, ensuring safety and comfort.
- Collaborate with healthcare teams to develop and implement individualized care plans.
- Document patient care accurately using EPIC electronic health record system.
- Support patient and family education regarding treatment and care plans.
- Maintain compliance with all healthcare regulations and facility policies.
Required Qualifications
- Valid and active Registered Nurse (RN) license.
- Experience working in LTC or SNF environments.
- Proficiency in EPIC electronic health records.
- Ability to work 12-hour shifts, including weekends and holidays.
Preferred Experience / Skills
- Strong clinical skills with elderly or geriatric patient populations.
- Excellent communication and teamwork abilities.
