Key Responsibilities
• Assist licensed occupational therapists in delivering personalized therapy services to patients in skilled nursing facilities.
• Implement treatment plans and therapeutic activities under supervision, ensuring patient progress and safety.
• Maintain accurate documentation of patient evaluations, progress notes, and treatment records.
• Verify and document patient eligibility, adhere to facility protocols, and ensure compliance with all licensing and credentialing standards.
• Support infection control protocols, including COVID-19 testing requirements prior to start date.

Required Qualifications
• Valid Occupational Therapy Assistant license in Oregon or applicable state.
• Current CPR certification.
• Ability to pass background checks including criminal, sex offender, and federal searches.
• COVID-19 test results within 72 hours prior to start date, with documentation.
• Successful credentialing verification including license, references, and background checks.

Preferred Experience / Skills
• Skilled nursing facility (SNF) experience preferred.
• Recent graduate candidates are encouraged to apply.
• Experience with healthcare documentation and adherence to regulatory requirements.
