Key Responsibilities

• Assist licensed therapists in implementing treatment plans for patients in skilled nursing facilities.
• Provide patient-centered physical therapy assistance under supervision.
• Document patient progress and communicate effectively with care teams.
• Ensure compliance with facility and regulatory standards.
• Maintain accurate documentation and patient records.
• Adhere to all safety and infection control procedures, including COVID testing requirements.

Required Qualifications

• Valid Physical Therapy Assistant license for Oregon.
• Certification or license verification must be provided upon submission.
• Ability to complete and provide evidence of recent COVID testing (within 72 hours prior to start).
• Strong communication and teamwork skills.
• Prior experience in skilled nursing facilities preferred.
• New graduates are welcome to apply.

Preferred Experience / Skills

• Skilled Nursing Facility (SNF) experience.
• Knowledge of infection control procedures related to COVID-19.
• Familiarity with credentialing and background check requirements.
