Key Responsibilities
• Provide patient care under physician orders, including treatments, procedures, tests, and basic care on an intermittent basis supervised by an RN.
• Collaborate as part of a multidisciplinary team to meet patient needs, demonstrating knowledge of growth, development, and psychosocial needs across age groups.
• Document and communicate patient conditions accurately, maintaining compliance with regulatory standards.
• Utilize medical and office equipment, including Point of Care devices, and operate within legal and safety guidelines.
• Assume accountability to the Care Manager regarding utilization practices and ensure adherence to compliance and safety standards.
• Participate in on-call rotations, including nights, weekends, holidays, and make home visits as needed.

Required Qualifications
• Valid LVN / LPN license in the state of Illinois.
• CPR certification prior to start.
• Own reliable transportation, valid driver’s license, and auto insurance, with adherence to safe driving practices.
• Ability to work independently and manage own workload efficiently.
• Knowledge of the nursing process and ability to collaborate with RNs on patient care.

Preferred Experience / Skills
• Recent nursing experience, preferably in home health care.
• Experience with Point of Care devices and home health-specific procedures.
• Ability to participate in flexible scheduling, including nights, weekends, and holidays.
