Key Responsibilities
- Provide physical therapy services in a Skilled Nursing Facility (SNF) setting.
- Develop and implement individualized treatment plans for patients.
- Monitor and document patient progress consistently.
- Collaborate with healthcare team to ensure optimal patient care.
- Adhere to all regulatory and safety standards.
Required Qualifications
- Valid California Physical Therapist license (must be currently active).
- At least 1 year of experience working in a Skilled Nursing Facility (SNF) preferred.
- Knowledge of NetHealth (Rehab Optima) is a plus.
Preferred Experience / Skills
- Experience working with rehabilitation software systems such as Rehab Optima.
- Strong clinical assessment and documentation skills.
