Key Responsibilities

• Provide outpatient physical therapy services for a home health patient population, including geriatrics and middle-aged adults.
• Manage a caseload of 5-6 patients daily, focusing on orthopedics, cardiac, neuro, pulmonary, and diabetic conditions.
• Conduct independent patient assessments, develop and implement treatment plans, and document progress using electronic charting systems.
• Supervise and collaborate with physical therapy assistants (PTAs) to ensure high-quality patient care.
• Manage time effectively, coordinate with the multidisciplinary team, and maintain patient-centered goals and professionalism.
• Comply with organizational policies, including scheduled weekend and holiday coverage, and adhere to safety and documentation standards.

Required Qualifications

• Valid state license to practice as a Physical Therapist in Illinois.
• BLS certification.
• Minimum of 1 year of clinical experience as a PT.
• Ability to work independently and as part of a team, demonstrating strong organizational and communication skills.

Preferred Experience / Skills

• Prior experience in home health settings.
• Knowledge of electronic health record systems (Epic preferred).
• Supervisory experience with PTAs.
• Flexibility and positive attitude toward patient care and team collaboration.
