Key Responsibilities
- Provide home health nursing care within a designated coverage area.
- Conduct in-person assessments and complete Oasis documentation.
- Manage patient care plans, including clinical visits and phone triage.
- Participate in on-call rotation and weekend coverage as scheduled.
- Collaborate with non-clinical staff to address patient needs effectively.
- Adhere to credentialing requirements, including in-person fingerprinting and DE licensure.
Required Qualifications
- Valid Delaware Registered Nurse license.
- Home health experience.
- Proficiency with Oasis documentation.
- Ability to travel within a 60-mile radius of Smyrna, DE.
- Willingness to plan 5 weeks ahead for credentialing process.
- Must be available for immediate start and adhere to scheduled orientation.
Preferred Experience / Skills
- Prior experience with on-call and weekend nursing coverage.
- Strong patient communication skills and ability to handle clinical phone triage.
- Reliable transportation within the coverage area.
