Key Responsibilities

• Provide home health nursing services within lower New Castle County and Kent County.
• Complete initial credentialing requiring in-person fingerprinting in Delaware.
• Manage patient care, including phone triage and home visits, primarily during weekdays.
• Participate in on-call duties evenings and weekends on a rotating schedule.
• Document patient care and ensure compliance with Oasis documentation standards.
• Collaborate with non-clinical staff to handle patient calls efficiently.

Required Qualifications

• Valid RN license in Delaware.
• Home Health experience and Oasis certification.
• Ability to plan approximately 5 weeks for credentialing process.
• Willingness to travel within a 60-mile radius.

Preferred Experience / Skills

• Prior experience working with Medicare or in a home health setting.
• Strong assessment and communication skills.
• Flexibility to work on call and weekend rotations.

• On-call rate: $7/hour, with pay for actual patient contact or triage.
• Travel RNs: On-call shifts from 4:30pm – 8:00am; weekend work once every 4 weeks.
• Cancel policy: 14 days’ notice required.
