Key Responsibilities

• Evaluate and assess patient conditions to develop appropriate treatment plans.
• Provide physical therapy services to patients in a skilled nursing facility setting.
• Monitor patient progress and adjust treatment plans accordingly.
• Document patient care and maintain accurate records.
• Collaborate with healthcare team members to ensure optimal patient outcomes.
• Comply with all licensing, safety, and documentation requirements.

Required Qualifications

• Valid state license as a Physical Therapist.
• Must include resume and license verification in application.
• Covid-19 test result within 72 hours prior to start date, with results issued within 48 hours of start.
• Ability to work in a fast-paced healthcare environment.
• New graduates are encouraged to apply.

Preferred Experience / Skills

• Previous experience in a skilled nursing facility (SNF) preferred.
• Familiarity with specific patient populations common in SNF settings is a plus.
