Key Responsibilities

• Provide social work services within an adult day care program, including one-on-one and group counseling.
• Conduct comprehensive social assessments, considering emotional, social, cognitive, and ethnic factors to develop individualized care plans.
• Access community and financial resources to support participants and ensure continuity of care across settings.
• Perform assessments and interventions in various settings, including participant homes, as needed, with some travel within the service radius.
• Collaborate with an interdisciplinary team to support participant needs, ensuring services align with the organization’s mission and values.
• Document client interactions and care plans using the clinical system provided.

Required Qualifications

• Valid WA license as an LSW, LSWAIC, or LASW (temporary licensure accepted).
• BLS certification.
• Minimum of 2 years of experience working with the geriatric population.
• Ability to perform assessments in community and home settings.
• Willingness to travel within the service area and manage flexible scheduling.

Preferred Experience / Skills

• Previous experience in adult day care or geriatric social work.
• Strong autonomous work ethic and ability to manage a flexible schedule.
• Good interpersonal and communication skills for counseling and team collaboration.
