Key Responsibilities

• Provide quality physical therapy services to patients in a skilled nursing facility.
• Develop and implement individualized treatment plans to meet patient needs.
• Monitor patient progress and adjust treatments accordingly.
• Maintain accurate documentation of patient progress and treatment procedures.
• Collaborate with healthcare team members to ensure comprehensive patient care.
• Adhere to all licensing, credentialing, and regulatory requirements.

Required Qualifications

• Valid State license as a Physical Therapist.
• Proof of license verification.
• Resume submission.
• Certification in Dementia Care (minimum of 8 hours).
• Completion of criminal background checks covering last 7 years, including state, federal, and nationwide searches.
• Social Security Number trace.
• Compliance with all credentialing and screening requirements listed in the credentialing checklist.

Preferred Experience / Skills

• Experience working in a Skilled Nursing Facility (SNF) highly preferred.
• Prior experience with multi-state criminal searches and background verification processes.
• Ability to work collaboratively within a healthcare team.
