Schedule / Job Type
- Full-time, Night Shift, 12-hour shifts
Key Responsibilities
- Provide licensed practical nursing care to residents in a long-term care and rehab setting.
- Monitor and record patient conditions and administer medications as prescribed.
- Collaborate with healthcare team to develop and implement care plans.
- Ensure compliance with facility policies and state regulations.
- Document patient care accurately in electronic medical records.
- Assist with patient assessments and provide comfort and support to residents.
Required Qualifications
- Valid LPN or LVN license in the state of Minnesota.
- Experience working in a long-term care or skilled nursing facility preferred.
- Knowledge of electronic medical records, specifically PCC.
- Strong clinical skills and attention to detail.
- Ability to work independently and as part of a team.
Preferred Experience / Skills
- Prior experience with rehab or general long-term care populations.
- Ability to work every third weekend and holiday shifts.
- Flexible with 12-hour night shifts.
