Key Responsibilities
• Provide social work services to participants in adult day care programs, including one-on-one and group counseling.
• Conduct comprehensive assessments considering social, emotional, cognitive, and ethnic factors to develop individualized care plans.
• Support participants by connecting them with community and financial resources, ensuring continuity of care across settings.
• Support participants through assessments and interventions in various environments, including some home visits within the service radius.
• Collaborate with interdisciplinary team members to deliver holistic care and ensure mission alignment.
• Document all services accurately in electronic health records (EPIC).

Required Qualifications
• Valid Washington State social work license (WA LICSW, WA LSWAIC, or WA LASW) at the time of submission. Temporary licensure accepted.
• Minimum of 2 years' experience working with a geriatric population.
• BLS certification.
• Ability to travel between multiple locations within the Seattle area, with mileage reimbursement.
• Strong assessment and communication skills, and ability to work independently with autonomy in scheduling.

Preferred Experience / Skills
• Experience working in outpatient or adult day care settings.
• Comfort conducting some assessments in participants' homes.
• Knowledge of community resources and care coordination.
