Key Responsibilities
- Provide social work services to participants at an adult day care program within a multidisciplinary team
- Conduct individual and group counseling sessions
- Perform comprehensive assessments addressing social, emotional, cognitive, and cultural factors to develop personalized care plans
- Connect participants with community and financial resources to support their well-being and ensure continuity of care
- Conduct assessments and interventions in participants’ homes as needed, supporting community-based care
- Maintain documentation and charting using electronic medical records (EPIC) system
Required Qualifications
- Active WA LICSW, WA LSWAIC, or WA LASW license (temporary licensure accepted)
- Minimum of 2 years of experience working with geriatric populations
- BLS certification
- Valid driver’s license and willingness to travel within the service area
Preferred Experience / Skills
- Experience working in outpatient or adult day care settings
- Ability to work independently with a flexible schedule
- Strong interpersonal and assessment skills
