Job Title
- Physical Therapist - Home Health
Location
- Oakland, CA
Schedule / Job Type
- Full-time, Day shifts, Onsite, Monday to Friday, with one weekend per month rotation
Key Responsibilities
- Prioritize start of care visits and ensure timely scheduling
- Supervise and support Physical Therapy Assistants
- Report start of care visits the following day
- Complete clinical notes and documentation within 48 hours of each visit
- Maintain effective communication with supervisors, intake, and office staff
- Provide care across the designated territory, which includes Marin, Sonoma, and Napa Counties
Required Qualifications
- Active California Physical Therapist license
- Minimum of one year of Home Health experience
Preferred Experience / Skills
- Experience working in a home health setting within Marin, Sonoma, and Napa Counties
- Ability to manage a varied caseload and coordinate multiple visits daily
