Job Title
- LVN / LPN - Long-Term Care / Skilled Nursing Facility

- Full-time, Mids shift (midday shift)
Key Responsibilities
- Provide direct nursing care to residents in a long-term care or skilled nursing facility setting.
- Assist with patient assessments, medication administration, and care plan implementation.
- Collaborate with interdisciplinary teams to ensure quality patient care.
- Maintain accurate documentation and comply with facility policies and procedures.
- Work on a bi-weekly rotating schedule, including every other weekend.
- Make up hours if call-offs occur on Friday, Saturday, or Sunday, per facility policy.
Required Qualifications
- Valid LVN or LPN license in the state of Pennsylvania.
- Certification in CPR and first aid.
- Experience in long-term care or skilled nursing facilities preferred.
- Ability to work independently and as part of a team.
- Strong communication and organizational skills.
Preferred Experience / Skills
- Previous experience with elderly patient care.
- Knowledge of electronic medical record systems.
