Key Responsibilities
- Provide high-quality nursing care to residents in a long-term care or skilled nursing facility setting
- Assist with patient assessments, medication administration, and care planning
- Collaborate with interdisciplinary team to ensure optimal resident outcomes
- Monitor and document patient conditions accurately and promptly
- Support residents with activities of daily living and foster a compassionate environment
Required Qualifications
- Valid LVN or LPN license applicable in the state of West Virginia
- Minimum of 1 year of experience in LTC or SNF settings preferred
- Strong understanding of nursing practices and patient safety protocols
- Excellent communication and teamwork skills
Preferred Experience / Skills
- Prior experience with electronic health records (EHR) systems
- Knowledge of state and federal healthcare regulations
- Ability to prioritize and manage multiple patient needs effectively
