- Full-time, Days shift, On-call weekend rotation required
Key Responsibilities
- Provide direct patient care in long-term care (LTC) and skilled nursing facilities (SNF) settings.
- Assess, plan, and implement individualized nursing care plans.
- Administer medications and treatments according to facility protocols.
- Monitor patient conditions and report changes to healthcare teams.
- Ensure compliance with healthcare regulations and safety standards.
- Document patient care accurately and promptly.
Required Qualifications
- Valid RN license in the state of New Hampshire.
- Strong knowledge of LTC/SNF clinical practices.
- Excellent communication and patient care skills.
- Ability to work effectively in a team environment.
- Flexibility to participate in on-call weekend rotations.
Preferred Experience / Skills
- Previous experience in long-term care or skilled nursing facilities.
- BLS and CPR certification.
- Knowledge of electronic health records (EHR) systems.
