Key Responsibilities

• Provide skilled physical therapy services to patients in a skilled nursing facility (SNF).
• Develop and implement personalized treatment plans based on patient assessments.
• Monitor patient progress and adjust therapy plans accordingly.
• Collaborate with a multidisciplinary team to ensure quality care.
• Maintain accurate documentation of all patient treatments and progress.
• Ensure compliance with regulatory and safety standards.

Required Qualifications

• Valid license and certification to practice as a Physical Therapist.
• Proven professional experience working in a skilled nursing facility (SNF).
• Knowledge of therapeutic techniques and procedures specific to SNF settings.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Excellent documentation and organizational skills.

Preferred Experience / Skills

• Experience with a variety of patient age groups and diagnoses.
• Familiarity with electronic health records (EHR) systems.
• Bilingual abilities are a plus but not required.
