Key Responsibilities
- Provide high-quality nursing care to residents in a long-term care or skilled nursing facility setting
- Administer medications, treatments, and supports according to care plans
- Monitor patient health and document observations accurately
- Collaborate with the healthcare team to ensure resident well-being and safety
- Assist with daily activities and provide compassionate patient support
- Follow all licensing, safety, and health regulations
Required Qualifications
- Valid LVN or LPN license in the state of New Hampshire
- Previous experience in long-term care or skilled nursing settings preferred
- Ability to work independently and as part of a team
- Strong communication and patient care skills
- Basic understanding of medical documentation and procedures
Preferred Experience / Skills
- Experience with geriatric or chronic care populations
- Knowledge of electronic health records (EHR) systems
