Key Responsibilities

• Evaluate and develop treatment plans for patients in skilled nursing facilities (SNFs).
• Provide physical therapy services to improve mobility, strength, and overall patient well-being.
• Document patient progress and adjust treatment plans as necessary.
• Collaborate with healthcare teams to ensure optimal patient care.
• Assess patient needs and educate patients and their families on therapy goals and methods.

Required Qualifications

• Valid state license or certification as a Physical Therapist.
• Minimum of 1 year of professional experience in a skilled nursing facility (SNF).
• Strong knowledge of physical therapy practices and procedures.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a team-oriented environment.

Preferred Experience / Skills

• Experience working with diverse patient populations.
• Familiarity with electronic health records (EHR) systems.
