Schedule / Job Type

• Days, 13 to 26 weeks contract, guaranteed 40 hours/week

Key Responsibilities

• Provide outpatient physical therapy services to patients in a clinical setting
• Document patient notes using Raintree EMR system
• Complete assigned pre-assignment modules prior to start, with reimbursement
• Maintain productivity levels of 55-63 patients per week
• Collaborate with team members to ensure quality patient care

Required Qualifications

• At least 3 months of outpatient physical therapy experience or 1 year of PT experience in other settings
• Ability to work day shifts
• Completion of Cypress Health Partners outpatient pre-screening form required with submission
• Willingness to complete pre-assignment modules (reimbursable upon completion)

Preferred Experience / Skills

• Experience with outpatient therapy in diverse settings
• Strong documentation and patient care skills
• Adaptability to various patient treatment plans
