- Full-time, Day shifts
Key Responsibilities
- Provide hospice admissions assessments and coordinate patient care
- Manage a caseload of geriatric patients, typically visiting up to 6 patients per day
- Document all patient interactions accurately using HomeCare HomeBase and Oasis systems
- Ensure timely communication and adherence to documentation standards
- Support patient and family education and comfort during end-of-life care
- Maintain compliance with all healthcare protocols and licensing requirements
Required Qualifications
- Active Washington state nursing license or compact license
- Minimum of 1 year of recent skilled home health experience within the last 3 years
- Basic Life Support (BLS) certification
- Valid driver’s license and own transportation
Preferred Experience / Skills
- Experience in hospice or palliative care
- Strong organizational and communication skills
- Ability to work independently within a team environment
