Key Responsibilities

• Provide social work services to participants in accordance with the facility’s mission and core values.
• Conduct one-on-one and group counseling sessions within the center.
• Perform comprehensive assessments covering social, emotional, cognitive, and ethnic factors to develop individualized care plans.
• Access community and financial resources to support participant needs and ensure seamless continuity of care.
• Conduct social work assessments and interventions in various care settings, including participant homes when necessary.
• Manage a flexible schedule for home visits, with travel occurring within the workday and reimbursed mileage.

Required Qualifications

• WA LICSW, WA LSWAIC, or WA LASW license (temporary licensure accepted).
• BLS certification.
• Minimum of 2 years of experience working with the geriatric population.
• Ability to perform assessments and provide social work interventions both within the facility and in the community.

Preferred Experience / Skills

• Experience in outpatient settings or working with geriatric clients.
• Comfortable with travel and autonomous scheduling of home visits.
• Strong interpersonal and communication skills.
