Key Responsibilities
- Provide skilled nursing care to residents in a long-term care or skilled nursing facility.
- Assist with medication administration and treatment plans under supervision.
- Monitor patient health and report any changes to the healthcare team.
- Document patient care and maintain accurate records.
- Collaborate with interdisciplinary team to ensure quality resident care.
Required Qualifications
- Valid Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license in the state of New Hampshire.
- Proven experience working in long-term care or skilled nursing settings.
- Good communication and documentation skills.
- Ability to work independently and as part of a team.
Preferred Experience / Skills
- Prior experience in a skilled nursing or long-term care environment.
- Knowledge of electronic medical records (EMR).
- Compassionate attitude and strong patient care skills.
