Key Responsibilities

• Assess, develop, and coordinate individualized care plans for residents in accordance with facility standards and procedures.
• Administer medications and treatments to assigned residents, ensuring accuracy and compliance.
• Observe and document residents’ conditions, reporting changes promptly to appropriate staff.
• Safeguard residents’ property, privacy, and confidentiality while maintaining their dignity and independence.
• Assist in maintaining safe, clean, and pleasant living environments for residents.
• Follow physicians’ orders accurately and notify healthcare providers in a timely manner.
• Use electronic medical records (EMR) effectively and safely operate transfer equipment such as gait belts, lifts, and slide boards.
• Provide education and support to residents and their families, addressing concerns and care plans.
• Respond promptly to resident call lights and provide compassionate, professional care at all times.
• Comply with all relevant laws, regulations, and organizational policies concerning nursing care and safety protocols.

Required Qualifications

• Graduate of an accredited nursing program.
• Valid RN licensure for the state of practice (Minnesota).
• Current BLS certification.
• Knowledge of basic nursing skills and medication administration.
• Ability to communicate effectively in English, both orally and in writing.

Preferred Experience / Skills

• Previous experience in long-term care or skilled nursing facilities.
• Familiarity with electronic health records (EHR/EMR).
• Knowledge of safety procedures, fire, and disaster protocols specific to healthcare environments.
