Key Responsibilities
• Provide outpatient physical therapy services to patients in a skilled nursing facility setting.
• Evaluate patient needs and develop individualized treatment plans.
• Implement therapeutic interventions to improve mobility, strength, and overall function.
• Document patient progress and update care plans accordingly.
• Collaborate with healthcare team members to ensure comprehensive patient care.
• Assist in patient assessments and discharge planning as needed.

Required Qualifications
• Valid state license or certification as a Physical Therapist.
• Proven experience in outpatient or skilled nursing therapy.
• Strong understanding of physiotherapy techniques and modalities.
• Excellent communication and interpersonal skills.
• Ability to work effectively within a team-oriented environment.

Preferred Experience / Skills
• Previous experience working in a nursing home or long-term care facility.
• Bilingual skills or additional language proficiencies are a plus.
