Key Responsibilities

• Provide skilled physical therapy services to patients in a skilled nursing facility (SNF).
• Assess patient condition, develop treatment plans, and implement therapy programs.
• Monitor and document patient progress, adjusting treatments as needed.
• Collaborate with healthcare team to ensure optimal patient outcomes.
• Adhere to all clinical guidelines and safety protocols.
• Maintain accurate documentation and complete credentialing requirements timely.

Required Qualifications

• Valid state license as a Physical Therapist.
• CPR certification.
• Ability to pass a background check, including criminal and sex offender searches.
• Compliance with all credentialing requirements, including license verification, references, and criminal background checks.
• COVID-19 test results within 72 hours prior to start date, with results issued 48 hours before start.

Preferred Experience / Skills

• Experience working in a skilled nursing facility (SNF) preferred.
• Recent graduate therapists are encouraged to apply.
• Knowledge of current therapy techniques and healthcare regulations.
