Key Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility
• Develop and implement individualized treatment plans
• Monitor patient progress and adjust treatments accordingly
• Document patient care activities and progress notes
• Collaborate with healthcare team to ensure optimal patient outcomes
• Maintain compliance with all regulatory and facility policies

Required Qualifications

• Valid state licensure as a Physical Therapist
• Relevant educational background and degree in physical therapy
• Ability to pass required background checks and screenings
• Current COVID-19 vaccination with test results within 72 hours prior to start and results issued within 48 hours

Preferred Experience / Skills

• Prior experience in skilled nursing facilities (SNF) preferred
• Recent clinical experience or new graduates acceptable
• Knowledge of documentation and regulatory requirements within SNF settings
