Key Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility setting.
• Conduct assessments, develop treatment plans, and implement therapy programs.
• Document patient progress and communicate with healthcare team members.
• Ensure compliance with all licensing, safety, and facility policies.
• Maintain accurate and timely records of patient care activities.

Required Qualifications

• Valid state license as a Physical Therapist.
• Ability to pass required credentialing and background checks.
• Current COVID-19 test results from within 72 hours prior to start date.
• Must complete and submit prescreening, resume, and license verification documentation with application.
• Willingness to work in a skilled nursing facility environment.

Preferred Experience / Skills

• SNF (Skilled Nursing Facility) experience preferred.
• New graduates are welcome to apply.
• Knowledge of dementia care and related therapies beneficial.
