Key Responsibilities
- Provide comprehensive hospice nursing care to patients in their homes.
- Assess patient needs and develop individualized care plans.
- Collaborate with interdisciplinary teams to ensure quality patient outcomes.
- Educate patients and families on disease management and care procedures.
- Document patient progress and interventions in compliance with healthcare standards.
- Travel within assigned service areas to deliver care as needed.
Required Qualifications
- Licensed Registered Nurse (RN) in the state of Indiana.
- At least 1.5 years of experience in a healthcare setting, preferably hospice or home health.
- Ability to work flexible hours, including some weekends and major holidays.
- Willingness to float to different locations and respond to changing patient needs.
Preferred Experience / Skills
- Previous hospice or palliative care experience.
- Strong assessment and communication skills.
- Ability to work independently with minimal supervision.
