Key Responsibilities
• Provide social work services to program participants, including one-on-one and group counseling.
• Conduct comprehensive assessments to develop individualized care plans that address social, emotional, cognitive, and ethnic factors.
• Access community and financial resources to support participants and ensure continuity of care across settings.
• Perform social work assessments and interventions in various settings, including participant homes and the outpatient center.
• Manage a flexible schedule for home visits, optimizing autonomy in visit planning while ensuring all travel occurs within the standard workday.
• Collaborate with an interdisciplinary team to support participant needs and program goals.
Required Qualifications
• BLS certification.
• Master’s Degree in Social Work (MSW).
• Valid Washington State Social Worker license (LICSW, LSWAIC, or LASW).
• Minimum of 2 years of experience working with a geriatric population.
• Ability to conduct assessments and provide services in community settings.
Preferred Experience / Skills
• Prior experience working with elderly or geriatric clients.
• Strong assessment, counseling, and resource coordination skills.
• Comfort with flexible scheduling and fieldwork within South King County.
