Key Responsibilities
- Provide rehabilitative therapy services to patients in a skilled nursing facility setting.
- Develop personalized treatment plans based on patient assessments.
- Monitor and document patient progress and adjust treatments accordingly.
- Collaborate with healthcare teams to ensure quality patient care.
- Ensure compliance with facility policies and healthcare regulations.
- Maintain accurate documentation and complete required credentialing when submitting candidates.
Required Qualifications
- Valid Physical Therapist license for the state of Massachusetts.
- State license verification completed and attached.
- Current resume attached.
- Successful completion of a COVID-19 test within 72 hours prior to start, with results issued 48 hours before start date.
- Ability to pass criminal background checks, including:
- County Criminal (7 years for all counties lived and worked)
- Federal Criminal (7 years or more)
- Multi-State Criminal Search (7 years or more)
- Nationwide Sex Offender Search (7 years or more)
- HHS/OIG Search
- GSA Search
- Social Security Number Trace
- Two-step PPD, Quantiferon, or proof of positive PPD and chest X-ray
Preferred Experience / Skills
- SNF (Skilled Nursing Facility) experience preferred.
