- Full-time, days shift, 35 hours/week, Monday through Friday
Key Responsibilities
- Provide outpatient physical therapy services to residents in an independent living, assisted living, and memory care setting
- Develop and implement personalized treatment plans to promote resident health and mobility
- Collaborate with an experienced rehab team to ensure quality care and progress monitoring
- Maintain accurate documentation of therapy sessions and patient progress
- Adhere to all regulatory standards and best practices in therapy
Required Qualifications
- Valid state license or certification as a Physical Therapist
- Proven experience providing outpatient therapy services
- Strong knowledge of rehabilitation techniques and modalities
- Excellent communication and team collaboration skills
Preferred Experience / Skills
- Experience working in senior living or memory care environments
- Ability to work independently with minimal supervision
