Key Responsibilities
• Provide skilled nursing care to residents in a long-term care or skilled nursing facility setting.
• Administer medications and treatments as prescribed, ensuring resident safety and compliance.
• Monitor and document patient conditions, reporting any concerns or changes to the healthcare team.
• Collaborate with team members to deliver quality patient care aligned with facility standards.
• Maintain accurate and up-to-date electronic medical records using PCC software.
• Adhere to facility policies, including weekend and holiday rotation requirements.

Required Qualifications
• Active MN LVN / LPN license.
• Active MN State Nurse Aide Certification (SNSA) registration.
• Prior experience in long-term care or skilled nursing preferred.
• Strong communication and patient care skills.
• Ability to work independently and as part of a team.

Preferred Experience / Skills
• Familiarity with PCC electronic health records system.
• Flexibility to work 8-hour or 12-hour shifts.
• Experience working weekend and holiday rotations.
