Job Title

• Full-time, Contract (13-week assignment), Days shift

Key Responsibilities

• Evaluate and treat residents to improve independence in activities of daily living (ADLs).
• Develop and implement individualized treatment plans tailored to each patient’s needs.
• Collaborate with interdisciplinary team members including physical therapists, speech therapists, nurses, and caregivers.
• Provide caregiver and family training to support safe and successful transitions.
• Document patient progress using electronic medical records for efficient workflow.
• Foster a positive, team-based, and compassionate therapy environment.

Required Qualifications

• Active Occupational Therapist (OT) license in good standing in Pennsylvania.
• Valid CPR certification.
• Active and clear state and federal background checks, including fingerprinting if not a resident of PA for two consecutive years.
• Ability to pass TB or PPD blood test, and provide MMR and Hep B status.
• Proficiency in documentation and communication skills.

Preferred Experience / Skills

• Experience in skilled nursing, rehabilitation, or older adult care.
• Strong problem-solving and interpersonal skills.
• Ability to work effectively within a team-oriented setting.
