Key Responsibilities
• Provide physical therapy services to patients in a Skilled Nursing Facility (SNF) setting
• Develop and implement personalized treatment plans to improve patient mobility and function
• Monitor and document patient progress, ensuring compliance with regulatory standards
• Collaborate with healthcare team members to deliver coordinated care
• Ensure all documentation and therapy sessions adhere to facility policies and state guidelines

Required Qualifications
• Valid state license in Physical Therapy
• Full vaccination for COVID-19 with negative test results within 72 hours prior to start date
• Ability to meet a productivity rate of 90%
• Professional demeanor and excellent communication skills
• Prescreen, resume, and license verification must be attached upon submission

Preferred Experience / Skills
• Prior SNF experience preferred
• Recent graduates are welcome to apply
• Familiarity with documentation procedures and therapy equipment
