Schedule / Job Type
- Full-time, rotating weekends, 12-hour shifts
Key Responsibilities
- Provide nursing care to patients with conditions such as dementia, Alzheimer’s, diabetes, and congestive heart failure (CHF).
- Adhere to established care plans and document patient information using electronic medical records (EMR) system.
- Support patient safety and comfort through medication administration and routine assessments.
- Collaborate with healthcare team members to ensure comprehensive patient care.
- Follow facility policies regarding patient care and safety protocols.
- Maintain a professional and compassionate approach to patient interactions.
Required Qualifications
- Active LVN or LPN license in the state of South Dakota.
- Experience working with elderly or chronically ill patients preferred.
- Basic knowledge of EMR systems, preferably Matrix.
- Ability to work 12-hour day shifts and rotating weekends.
- Good communication and teamwork skills.
Preferred Experience / Skills
- Prior experience in long-term care or skilled nursing facilities.
- Familiarity with managing dementia, Alzheimer’s, or chronic illnesses.
