Key Responsibilities
- Provide skilled nursing care to residents in a 66-bed long-term care facility.
- Monitor and manage patients with diagnoses such as CHF, dementia, DM, CKD, COPD, dysphagia (PEG tube), urinary retention (Foley catheters), and wounds.
- Collaborate with a team following patient care plans and document using PCC electronic medical records.
- Follow facility-specific orientation procedures by shadowing experienced staff for one shift.
- Maintain appropriate patient ratios, currently 22-25 residents per nurse, while supporting low census operations.
- Ensure compliance with all safety and care standards, including weekend and holiday coverage as scheduled.
Required Qualifications
- Valid LVN or LPN license in the state of North Dakota.
- Experience working in long-term care or skilled nursing settings.
- Familiarity with electronic medical records, specifically PCC.
- Ability to work 8-hour shifts, with current staffing including AM and Night shifts.
- Good communication skills and the ability to follow care plans accurately.
Preferred Experience / Skills
- Prior experience with managing complex chronic conditions such as CHF, COPD, and wounds.
- Experience working with residents requiring dysphagia management, PEG tubes, or Foley catheters.
- Ability to adapt to facility protocols and handle a variety of patient needs efficiently.
