Key Responsibilities
- Provide skilled nursing care to patients in their homes within a home health agency.
- Conduct assessments and develop tailored care plans for patients.
- Perform OASIS documentation accurately and efficiently.
- Manage patient cases from admission through discharge, coordinating with healthcare teams.
- Educate patients and families on health management and post-treatment procedures.
- Ensure compliance with all regulatory and safety standards.
Required Qualifications
- Valid Registered Nurse (RN) license.
- Experience in home health nursing required.
- Strong knowledge of OASIS documentation.
- Prior case management experience preferred.
- Ability to work independently in a home setting.
- Must have no requested time off during the contract period.
Preferred Experience / Skills
- Travel nursing experience highly desirable.
- Excellent patient communication skills.
- Ability to adapt quickly to diverse patient needs.
