Key Responsibilities
• Provide comprehensive physical therapy services to patients in various clinical settings.
• Assess patient needs, develop treatment plans, and implement therapeutic interventions.
• Collaborate with healthcare teams to ensure optimal patient outcomes.
• Document patient progress accurately and maintain up-to-date records.
• Comply with all facility policies, including drug-free workplace requirements.
• Adhere to local policies, including call-off and orientation guidelines.

Required Qualifications
• Valid Illinois State license as a Physical Therapist or confirmation that licensure application has been initiated.
• Active license or authorization to practice in Illinois.
• Ability to produce a negative drug screen for all panels.
• Last four digits of Social Security Number.
• Middle initial.
• Knowledge of or willingness to adhere to OSF policies and procedures.

Preferred Experience / Skills
• Previous outpatient or acute care experience preferred.
• Strong assessment and treatment planning skills.
• Excellent communication and teamwork abilities.
