Key Responsibilities

• Evaluate and treat patients using medically prescribed physical therapy programs to restore function and prevent disability.
• Document clinical observations, interventions, goals, and discharge plans accurately.
• Make initial admission/evaluation visits and communicate findings to the clinical team within established timelines.
• Supervise Physical Therapy Assistants as required.
• Collaborate with the clinical team to develop and adjust treatment plans aligned with patient needs.

Required Qualifications

• Valid and current license as a Physical Therapist.
• At least one year of home health experience.
• Knowledge of medically prescribed physical therapy practices.
• Ability to assess patient needs and develop appropriate treatment plans.
• Excellent documentation and communication skills.

Preferred Experience / Skills

• Experience working in community or home health settings.
• Familiarity with OASIS documentation and scoring.
