Key Responsibilities

• Provide physical therapy services to patients according to physician orders and standards of care.
• Complete necessary documentation directly with the appropriate health system prior to start.
• Collaborate with team members to develop and implement individualized treatment plans.
• Ensure compliance with licensing, equipment, and credentialing requirements.
• Participate in credentialing processes, including documentation and license verification.
• Maintain accurate records and documentation for billing and compliance purposes.

Required Qualifications

• Active state license as a Physical Therapist.
• Experience in setting and outpatient care preferred.
• Ability to complete required documentation accurately and timely.
• Equipment provided during interview.
• Must meet credentialing requirements (contract QA).

Preferred Experience / Skills

• Previous experience working in a similar setting or agency.
• Knowledge of credentialing processes and documentation standards.
• Strong communication and team collaboration skills.
