Key Responsibilities
• Provide skilled physical therapy services to patients in a skilled nursing facility (SNF).
• Develop and implement individualized treatment plans based on patient assessments.
• Monitor patient progress and adjust treatments as necessary.
• Educate patients and staff on therapeutic exercises and proper care techniques.
• Document patient evaluations, treatments, and progress in accordance with facility policies.
Required Qualifications
• Valid state license as a Physical Therapist.
• Minimum of 1 year of professional experience working in a Skilled Nursing Facility (SNF).
• Strong knowledge of physical therapy principles and practices.
• Excellent communication and patient care skills.
Preferred Experience / Skills
• Experience with elderly or long-term care populations.
• Ability to work effectively within a team environment.
