Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting.
• Manage an average of 5-20 patient visits per day, ensuring quality care.
• Collaborate with team members and adapt to facility needs, including occasional scope floating.
• Document patient care accurately using electronic health records.
• Participate in on-call responsibilities as required.
• Follow all safety and procedural protocols to ensure patient well-being.

Required Qualifications

• Valid state license as a Physical Therapist (or pending license accepted).
• Minimum of 1 year of home health experience.
• BLS certification.
• Willingness to work every 6th weekend.
• Ability to travel within a territory radius of 40-80 miles; mileage reimbursed at $0.55/mile.

Preferred Experience / Skills

• Prior experience working in home health or outpatient physical therapy.
• Ability to work independently and as part of a multidisciplinary team.
