Key Responsibilities
• Provide skilled nursing care to an assigned caseload of patients in their home environment
• Monitor and evaluate patient care, treatment, and collaboration with interdisciplinary team members to ensure optimal health outcomes
• Educate patients and families on proper home health care procedures and strategies
• Perform comprehensive assessments, including weekly wound measurements and accurate documentation in EMR systems
• Review and revise the plan of care in collaboration with the Home Health RN Care Manager based on patient progress
• Maintain current medication records and ensure adherence to physician orders
• Communicate regularly with the care team regarding patient status, barriers to care, and progress toward goals
• Participate in performance improvement initiatives and interdisciplinary team meetings
• Participate in on-call rotation to provide after-hours support
Required Qualifications
• Associates Degree in Nursing (ASN) or higher
• Valid Registered Nurse license in Alabama or a Compact state (no temporary licenses accepted)
• AHA BLS or Red Cross Basic Life Support certification
• Proficiency with NURSYS EMR and Homecare Homebase (HCHB) systems
• Minimum of 1-2 years of nursing experience, with at least 1 year in a specialized home health setting preferred
• Ability to lift up to 50 pounds and maneuver patients over 200 pounds, with good manual dexterity
• Valid driver's license and ability to operate a motor vehicle
Preferred Experience / Skills
• Experience in wound care, medication management, and patient education in a home health setting
• Ability to assess and improve patient outcomes, including participating in quality performance initiatives
• Strong communication and team collaboration skills
