Key Responsibilities
- Provide hospice care services in a home setting to patients requiring end-of-life support.
- Manage and coordinate patient care plans, ensuring compassionate and comprehensive care delivery.
- Work independently in a rural environment, traveling to patient homes as needed.
- Document patient interactions and care activities accurately using electronic medical records.
- Collaborate with interdisciplinary teams to meet patient needs and family support requirements.
- Maintain compliance with hospice regulations and agency protocols.
Required Qualifications
- Valid Registered Nurse (RN) license.
- Prior experience with hospice case management.
- Proven experience providing hospice care in a home setting.
- Ability to travel in rural areas comfortably and efficiently.
- Experience with electronic medical record systems, preferably McKesson/Netsmart.
- Flexibility to work Sunday through Thursday schedule.
- Candidates with no RTO (Request for Time Off) during this contract period preferred.
Preferred Experience / Skills
- Experience working in rural or home care environments.
- Familiarity with McKesson or Netsmart EMR systems.
- Strong communication and compassionate caregiving skills.
