Key Responsibilities

• Assist patients and families in navigating healthcare resources and discharge planning.
• Adapt to floating assignments across different units within the facility as needed.

Required Qualifications
• Valid social worker license or certification.
• Hospital or healthcare facility experience is mandatory.
• Acute care experience is required.
• Experience working in a hospital setting with a focus on acute care.
• Upfront submission of all return-to-office (RTO) documentation.

Preferred Experience / Skills
• Experience working in Laguna or surrounding facilities.
• Knowledge of local healthcare regulations and protocols.
• Ability to handle multiple cases and adapt to a floating assignment schedule.
